After you wrote that you get also just empty page when running check.php I don't believe this will help.

Instead of going on with 1.2.0 you could check out latest nightly build or wait until 1.2.1 is vailable (I think pretty soon)

You could try the latest nightly build, or wait until 1.2.1 is available (I think this will be the next few days)
There are fixes included related to URL-paths
I am pretty sure that your problem is also touched by the changes.
If not, please enter an issue for this.

Installation should be no problem.
Just copy the contents to the plugin folder of MantisBT and use the plugin administration page to activate the plugin.
Customization might be more difficult (I have no own experience) but I got the impression that the author of the plugins provides good support.

For real production purposes I use Linux.
When testing Windows I use XAMPP package.
