Summary0009918: Revise the header in the source code.
Description

The headers to be revised:

  1. The header for the standard MantisBT code.
  2. The header for MantisConnect.

Issues to consider:

  1. Use # vs phpDoc format.
  2. Update the copyright to 2009 or use @@YEAR@@ which is replaced via the build / packing script.
  3. Fix the indentation, in a lot of files we have to header blocks each with a different format / implementation.
