custom field not showing in "enter report details"

I added a custom field but it doesn't show up in Enter Report Details, nor in the edit details page. I found out how to configure the "view issues" page to show it plus a few other places. How do I get it to show in the create and edit pages?

Along the same lines, I'd like to remove fields I don't use from these two pages.

(added 12oct) I've reinstalled mantis on top of my existing installation and this problem still persists. I removed my changes (other than admin level stuff) from config_inc.php. I can try to add a custom field, check all the checkboxes where it is supposed to appear, and it appears nowhere.
